There are three directions of syncing data between Mac and mounted storage devices - Mac to mounted device (Mac data will be added to the existing entries on mounted device), mounted device to Mac (mounted device data will be added to the existing Mac entries) and bi-directional - data will be mutually updated on both sides.

Folders can be synced in three directions, iTunes & Video and iPhoto can be synced in two directions only (mounted device to Mac and Mac to mounted device).
